I have had a bit of a run around on this (sigh!).  The information available, as I was told by somebody at Isleworth Recreation centre today - is a bit of a mystery.I (now) understand that Dr's have the referral forms and these are sent off to a NHS representative for application to join the 8 week programme which is run though CIP at a couple of centres dotted around the Borough including Isleworth.  The form which I have had sight of suggests that the programme can include Swimming and exercise classes - I was told on the phone on Saturday that it was only for the gym.This is the information on the scheme received via e-mail a short while ago.Run through CIP - Hounslow PCTPart of a Healthy Lifestyles ProjectCalled "Exercise prescription"*  8 weeks use of the gym, swimming pool and/or exercise classes. *  Use of the facilities under guidance following health screen and GP’s/Health worker’s referral.              *  Overall aim is to empower and motivate the individual to make informed choices and improve his/her health psychologically, physically and socially.*  To raise awareness on health issues *  To gain knowledge regarding health*  To motivate the client through the use of exercise.*  To be able to identify areas of need and help meet the health needs through exercise.*  To increase clients body and mind awareness*  To develop skills and experience of an exercise and activity nature.*  To allow individuals to experience different and appropriate forms of exercise.*  To develop health/fitness and a more active and positive life*  Develop appropriate exercise and health plan for the client *  To provide a support network that encourages continuation of planObjectives are -*  To help develop independence*  To reduce dependency on surgery’s, health centres and medication            *  To develop a range of coping and life skills *  To increase interest in activities and facilities in the local area*  To improve the general level of health of the clientProcess is -*  GP/Health worker assesses appropriateness of client for the course.*  GP/Health worker fills in referral form and sends it alongside any relevant information to the Lifestyle Officer.*  Lifestyle officer contacts client to arrange date for health assessment and induction to the gym.*  Client will be charged £2.50 per session. He or she must be committed to maintain the 8-week course and an independent follow on program of a further 8 weeks is also offered at £2.50 per session once the initial program is complete.    Programme is -         *  The client after receiving the health assessment and attended the induction is given an appropriate exercise programme *  The client will attend on a regular basis to carry out their programme under qualified supervision.*  The client will express any concerns to the instructor who will evaluate the progress of the client and feedback any relevant information to the Lifestyle officer, Health worker or GPREVIEWAn evaluation will be made in conjunction with the clients as to the effectiveness of the programme and if agreed, a review will be made for a later date to help monitor any progress or lapses.Any reports on the clients’ development will be sent to the relevant GP on request.
