As I said on Chiswick W4, personally I think none of us should jump to conclusions about the cause and should let the official enquiries run their course.But it must be remembered that accidents do happen and no matter what anyone does they will continue to happen until the end of time, they are an inevitable part of day to day life, and just occasionally and very sadly with tragic consequences.

This is an absolute nightmare that should never have happened.  I only hope that once the causes are uncovered it gets as much attention as this horrible accident, and something is done immediately to rectify whatever went wrong.

The poor parents must be in a hell of a state, what could be worse than losing a child? I think everyone is thinking of them at this terrible time.
